AUSA 2024- FAMILY TOPICS
The Association of the United States Army (AUSU) will host its annual conference 14-16 October 2024 in Washington, D.C.  On the family side, two Family Forums and a Fireside Chat are planned.  All are invited to attend in person. Digital links from live-stream and recordings will be posted on the AUSA site.

TIP!  PARENTS OF CHILDREN 12 & UP ACCESSING CHILD’S MEDICAL INFORMATION
1.  DD FORM 2870 PURPOSE:   This form allows the Military Treatment Facility/Dental Treatment Facility/TRICARE Health Plan to legally disclose an individual's protected health information.  Some health information is protected beginning at age 12 and this form can be important for parents to understand.          

2.  WHY IS IT IMPORTANT?

DEPENDENTS OVER 18 must give authorization for anyone (even the parent!) to access their medical records. 
The child can either fill out DD Form 2870 and turn it to their hospital’s Patient Administration Division (PAD) or create a  DS LOGON and access their electronic records in MHS Genesis.
-DEPENDENTS 12-18 must give authorization for anyone (even the parent!) to access records or appointments that deal with substance abuse, sexual health or behavioral health.  Children 12 to 17 years old cannot create a DS Logon 
so DD Form 2870 is the form you must complete and turn in with your child. 

3.  WHAT DOES A PARENT DO WITH THIS FORM?  
-FOR MILITARY HOSPITALS OR CLINICS- complete it, child signs it, bring child with you to the Patient Administration Division (PAD) to turn it in.  You must bring the child with you.
-FOR OFF POST MEDICAL FACILITIES- Ask about their procedures for you to gain access to a restricted portion of your child’s medical records/appointment.
